DaVita (NYSE:DVA) has expanded its partnership with Humana (NYSE:HUM) through a new value-based care agreement covering more than 10,000 Medicare Advantage members living with chronic kidney disease.
The programme, which launched on July 1, targets patients with stages 3B through 5 chronic kidney disease (CKD). It builds on an existing relationship between the two companies that previously concentrated on patients with end-stage kidney disease, extending coordinated support to people at earlier stages of the condition.
DaVita IKC to Coordinate Patient Care
Services will be provided through DaVita Integrated Kidney Care (DaVita IKC), which brings together care across kidney, cardiovascular and metabolic health.
By engaging patients before they reach kidney failure, the programme aims to help preserve kidney function, slow disease progression and reduce hospitalisations. The value-based model is designed to coordinate treatment across different healthcare needs rather than addressing kidney disease in isolation.
Patients whose condition progresses to kidney failure will also receive support, including information about home dialysis and kidney transplantation.
Additional assistance will cover areas such as nutrition, transportation and mental health, reflecting the programme’s broader approach to managing factors that can affect patient outcomes.

Partnership Extends DaVita’s Value-Based Kidney Care Network
DaVita said the programme builds on its network of approximately 3,000 nephrologist partners participating in value-based care arrangements.
The expanded Humana agreement increases the emphasis on earlier intervention, with the companies seeking to improve outcomes before patients reach the most advanced stages of kidney disease.
As of June 30, 2026, DaVita served approximately 298,500 patients through 3,266 outpatient dialysis centres worldwide. Its network included 2,671 centres in the United States and another 595 facilities across 14 other countries.
The latest agreement strengthens the existing relationship between DaVita (NYSE:DVA) and Humana (NYSE:HUM), while broadening their coordinated kidney care model to a substantially larger group of Medicare Advantage patients.
